Most review templates are a blank HR form. The Manager's Performance Review Kit is for the person doing the writing. It follows four steps for each person on your team: gather the evidence, write the review, hold the conversation, and follow up.
The Evidence Log and Review Prep page collect what actually happened during the year, so the review isn't built from the last two weeks. The Review Writing Worksheet walks you through the summary, results, strengths and growth areas, each with an example. When you get stuck on wording, the two-page Phrase Bank has more than 90 lines: fill-in-the-blank sentences across 8 skill areas, lines for each rating level, and "instead of... write..." swaps that turn vague words into evidence.
The Year-End Work Review Kit is for the person being reviewed. In three sittings of about 45 minutes you look back on the year, write your self-assessment, and plan the next one. It includes a Wins & Brag Log, sentence starters for the questions most review forms ask, a STAR Story Builder, and a Review Meeting Prep page for your ask: a promotion, a raise, new scope or training.
If you want help with the writing itself, read how to write a self-evaluation, with example answers.
Many managers are reviewed while they review their team. The Performance Review Bundle has both kits for less than buying them separately. New to managing? The New Manager Toolkit adds 1:1 agendas, meeting notes, team goals and a task tracker to the review kit.
The templates

Gather the evidence, write a fair review for each person, hold the conversation and follow up at 30, 60 and 90 days.

A self-assessment kit: turn this year's work into a self evaluation you can stand behind, go in with a clear ask, and set 2027 goals.

Both review kits: the Year-End Work Review for your own self assessment and the Manager's Kit for your team.

Five tools for a new manager: 1:1 agendas and meeting notes, team goals with OKRs, a task and promise tracker, priorities, and the full performance review kit.
